Getting Started at Our Office
Get Started – We look forward to your visit, and we want you to look forward to it, too! In order to prepare for your initial visit, we want to share some information to ensure you come prepared, informed, and have the best experience possible!
Here’s what to expect on your first visit:
> Get Started Step 1: New Patient Intake Form- Fill this out beforehand to save time
Get Started – In order to utilize your time well, we’d love for you to fill out our new patient form(s) before coming in for your first appointment.
New Patient Intake Form- (links to adults and children forms.. will get these documents to you ASAP)
> Step 2: Consultation and Exam
Your initial visit starts with your consultation, where Dr. Isaiah will perform a detailed examination and then specific neurological-based testing. This helps us determine the best course of action for your treatment. Throughout your treatment plan, we will perform follow-up exams so we can map your progress and improvements.
> Step 3: X-rays & testing
We may take x-rays for diagnostic purposes. However, if you’ve had x-rays recently, we might be able to use your previous images if they cover the areas needed for proper diagnosis. If you have any questions or would like help transferring your x-rays to our office, give us a call!
> Step 4: Relief Treatment & Follow Up
Following your initial visit at Champion Health Chiropractic, will be the Doctor’s Report of Findings. This appointment includes a detailed interpretation of your testing results and the doctor’s recommendations on treatment and frequency of care.
